Below is the request form for Gift of Wellness gift certificates. Filling out the form below does NOT purchase a Gift of Wellness gift certificate, your answers will be submitted to our Membership Department and they will contact you with details about your gift certificate to complete the transaction.
Gift certificates valid for one use. Not valid on multi-session packages or tennis packages. Certificates must be redeemed by 6/30/21 & are only valid for the service & time specified. Limited to 3 gift certificates purchased per person.
